We have combined a Patient Registration Package for your convenience. It includes our most commonly used forms that will provide us with the information we need to address your snoring in the proper manner. Please download the entire package, complete the information on each form and return them to our office at your earliest convenience. We ask that you be forthcoming regarding your medical history, medication and other pertinent information. Having a complete medical history gives our providers a better perspective in regards to your specific treatment plan. Your records are kept in strict confidence.
